He was predeceased by: his parents, John Herbert and Pauline Herbert (Habrun); his sons, Allen Herbert and Daryl Herbert; and his siblings, Jeanette Herbert, Richard and Ron Herbert.
He is survived by: his wife Sally A. Herbert; his sister Linda Weaver (Gary); his children, Cheryl Perkins (Rick) and Troy Herbert (Trina); his daughter-in-law Ann Herbert; his grandchildren, Michael Herbert, Joshua Herbert (Megan), Holly May (Ron), Scott Herbert (Rachel), Andrew Perkins (Cassie), Chelsea Moauro (Matt) and Amy Small (Kurt); and his great grandchildren, Marlee, Colton, Mila Herbert, Evan, Nora May, Kenleigh, MJ, Harmony, Cohen Perkins, Laken, Myles Moauro and baby Small.
