He was predeceased by: his parent Dale T. Evans. He is survived by: his mother Jennifer E. Evans (Young); his daughters, Maddie and Madison; his son-in-law Kyle Kelley; his sister Melissa; his brother-in-law Michael Madaffer; and his sibling's children, Lyndsay, Caroline Madaffer and Evan Madaffer.
