He was predeceased by: his parents, John Kokolus and Helen Kokolus (Micio). He is survived by: his son Ty; and his brothers, John Kokolus and Allen Kokolus (Annie). He is also survived by several nieces, nephews, aunts, uncles, and cousins.

In lieu of flowers, memorial contributions may be presented to St. Jude Children's Research Hospital in loving memory of Tony.
