He was predeceased by: his parents, Gerald Dwayne Puckett and Sarah Ann Henley. He is survived by: his wife Gina Puckett of Jackson, Tennessee; his children, Bryan Scott Wilson (Rhonda) of Cedar Grove, Jeremy Dwayne Puckett (Angela) of Milan, Jeff Wilson (Kim) of Medina, Amanda Trull (Bishop James) of Gleason and Tonya Gooch (Gaylon) of Medina; and his siblings, Roy Lynn Puckett (Carol) of Cades, Tennessee and Anne Arrington (David) of Trenton, Tennessee. He is also survived by six grandchildren; and eleven great grandchildren.

In lieu of flowers, the family requests donations be made to the Alzheimer's Association.
