He was predeceased by: his parents, Leland Forrest and Mabel Forrest (Snell); his siblings, Kenyon Forrest, Donna Johnston (Warren) and Diane Misener (Don); and his close friend Ellen Decent.
He is survived by: his former wife Barbara L. Forrest; his children, Matthew Forrest and AmySue Hartman (Tom Korn); his grandsons, Jesse and Jacob Hartman; and his sister-in-law Joan Forrest. He is also survived by nieces and nephews, and many very special friends.
