He was predeceased by: his parents, Floyd Hazen and Ella Hazen; his wife Dorothy Eschenbauch; his son Keith Hazen; and his sister Patsy Hazen.
He is survived by: his children, Patricia Novotny (Brian), Lianne Rabehl (Lance) of Beloit, WI, Helene Burke (John) of Rayville, LA and Eric Hazen of Oshkosh, WI; his stepsons, Donald Eschenbauch (Lora) and Darin Eschenbauch (Jill); and his close friend Pearl "Dumpling Girl". He is also survived by six grandchildren and four great-grandchildren.
In lieu of flowers, a memorial has been established.
