He was predeceased by: his mother Betty England. He is survived by: his father Thomas Stevens Sr. of Crum, WV; his wife Angela L. Chapman of Huntington, WV; and his siblings, Regina Hardwood of Brownstown, MI and Thomas Stevens of Crum, WV. He is also survived by three children and one grandchild.
