He was predeceased by: his father William Joseph Polan. He is survived by: his siblings, Robert Shane Polan (Suzanne), Benjamin F. C. Polan, Jon Robert Voitelle (Carrie) and Chase M. M. Polan Chamberlain (Chris) of Arizona; his aunt Lynn; and his uncle Bob Fischer.
