He is survived by: his son Michael Cooper of Lexington, KY; his stepson Daniel Cook (Helen) of East Berlin, PA; his grandchildren, Mandi Cook (Jason), TJ East Berlin (Christy) and Kate Johnson (Mike) of Houston, TX; his great grandchildren, Ian, Braiden Cook and Alexander Johnson; and his caregivers, Mel and Becky Davis of Barboursville.
