He was predeceased by: his parents, Russell Blake and Ruby Blake; his son Michael Blake; his grandson Derek Alan Jeffers; and his sister Diana "Katie" Hodges.
He is survived by: his wife Janice Rowsey Blake of Huntington; his children, Christopher Blake (Jaime) of Huntington, Timothy Blake of Huntington and Deborah Kay Metzger (Donnie) of Huntington; his grandchildren, Megan Cullum (Peach), Amanda Blake, Morgan Blake, Hannah Blake, Landon Blake, Robert Blake, Alexis Taylor (Hunter) and Lillyan Nechovski; his great grandchildren, Derek Jeffers, Jr., Haley Beverly (Hunter), Roman Williams, Jaidyn Hill and Matthias Taylor; his brothers, Danny Blake (Jody), Dobie Blake and Michael Blake; and his brother-in-law Tom Rowsey (Carla). He is also survived by nieces and nephews.
In lieu of flowers, the family requests donations be made to St. Jude's Children's Hospital in his memory.
