He was predeceased by: his parents, Michal Foltak and Maria Pilat. He is survived by: his wife Krystyna Perkowska; his sons, George Foltak (Katarzyna Slawek) and Richard Foltak (Abra Foltak); his grandchildren, Melanie, Roger, Andrew, Gabrielle and Ava; and his sister Stanislawa Palczak. He is also survived by his nephews and nieces, cousins, and many other relatives and friends.
Visitation will be held on Monday, October 13th 2025 from 6:00 PM to 9:00 PM at the Urgel Bourgie Funeral Complex (6700 Rue Beaubien E, Montréal, QC H1M3E3). A funeral service will be held on Tuesday, October 14th 2025 at 1:30 PM at the Our Lady of Czestochowa Catholic Church (2550 Av. Gascon, Montréal, QC H2K3A2). A burial will be held on Tuesday, October 14th 2025 at 3:00 PM at the Repos Saint-François D'Assise Cemetery (6893 R. Sherbrooke E, Montréal, QC H1N1C7).
